- Should you find filling out online form too difficult, please let our scheduling staff know and we will promptly mail you a paper copy.
- We kindly ask for you to arrive 10-15 minutes prior to your appointment time so that our staff can review provided information and have your chart ready by your appointment time.
- If you have medical insurance, please bring your current insurance card and valid photo identification card with you at the time of your appointment.
